What is Failure Mode Effect Analysis in Hindi (FMEA क्या होता है)?
Failure Modes को पहचानने की सोच सैकड़ों सालों से प्रयोग मे लाई जाती थी लेकिन इसका सबसे पहले लिखित प्रयोग aerospace industries मे NASA ने 1960 के Apollo प्रोग्राम के समय FMEA को प्रयोग किया था।
NASA ने सोचा अगर हम पहले ही सभी failure modes को पचान ले और उन पर action ले लिया जाए तो हमारी success की संभावना बढ़ जाएगी।
इसके बाद 1970 के दशक मे automotive industries ने इसे प्रयोग करना शुरू किया और शुरुआती समय के quality management system थे QS-9000 और APQP 1994 मे FMEA को system की तरह जोड़ा गया था। और FMEA की सफलता को देखते हुए सभी industries ने इसका प्रयोग करना शुरू कर दिया है।

- Identifies Design or process related Failure modes before they happen.
- Determine the Effect & Severity of these failure modes.
- Identifies the causes and probability of occurrence of the failure modes.
- Identifies the control and their Effectiveness.
- Quantifies and priorities the Risks associated with the failure modes.

What is Failure Mode Effect Analysis Terminology (FMEA के Terminology क्या होता है)?
- Failure Modes:- Failure Modes का मतलब हुआ की मेरे प्रोसेस मे failure होने की संभावना क्या है और किस तरह से मेरे प्रोसेस मे failure हो सकते है।
- Failure Modes Effect:- Failure Modes Effect का मतलब होता की प्रोसेस मे अगर failure होता है तो उसका effect क्या होगा।
- Severity Ranking:- Severity Ranking का मतलब होता है की अगर प्रोसेस मे फैल्यर हुआ तो उसका impact कितना Sevier (गंभीर) होगा।
- Failure Mode Causes:- Failure Mode causes का मतलब होता है की failure होने के कारण क्या-क्या हो सकते है।
- Occurrence Raking:- यह एक अंदाज है की अगर failure हुआ तो उसके occurrence की probability की frequency कितनी होगी।
- Failure Modes Control:- इसका मतलब है Failure modes को control करने के लिए अभी मेरे पास क्या सिस्टम/mechanism/method/test process है।
- Detection Ranking:- Detection Ranking का मतलब है की अगर failure होता है तो detection के प्रोसेस मे कितनी probability है की failure को पकड़ लिया जाएगा।
- Risk Priority Number(RPN):- यह Severity, Detection और Occurrence का गुणा होता है। RPN=S×O×D
- Action Planning:- इसमे हम action plan करते है की मेरे जो अलग-अलग एलीमेंट है Severity, Detection और Occurrence है इनमे improvement करके कैसे risk को कम कर सकता हूँ।

Step 5:-Determine Causes
Causes को पहचानने के लिए 6M पर जाएं।
Step 6:-Current Control
यहाँ हम देखते है की हमारा वर्तमान control क्या है failure को Prevent और Detect करने के लिए।

Step 7:-Calculate Risk
यहाँ हम RPN number को calculate करते है।
Step 8:-Take Action
इस स्टेप मे हम RPN को कंकरने के लिए एक्शन लेते है।
Step 9:-Assess Result
इस स्टेप मे हम देखते है की corrective action लेने के बाद मे RPN मे क्या कमी आई।

FMEA की गणना कैसे की जाती है?
FMEA में दर्ज की गई जानकारी की गणना की जाती है, और आउटपुट के रूप में जोखिम प्राथमिकता संख्या (RPN) संख्या प्राप्त होती है। RPN की गणना प्रत्येक मान्यता प्राप्त विफलता मोड की गंभीरता, घटना और पहचान की संख्या को गुणा करके की जाती है (RPN = गंभीरता x घटना x पहचान)।
